For a country of 173.6 million, that is a total workforce of roughly 11,500 clinicians for the entire nation. Fewer than 300 are psychiatrists, and the majority practice in Dhaka. Divisional cities like Chattogram, Sylhet, and Rajshahi have a handful each; the country's more than 460 upazilas typically have none.
The visible symptom of that shortage is a 12-week wait for a first appointment at the National Institute of Mental Health in Dhaka, at Bangabandhu Sheikh Mujib Medical University, or at a divisional medical college's psychiatry outpatient clinic. Private consultations in Dhaka's Gulshan or Dhanmondi neighborhoods run at rates that a lower-middle-income household, Bangladesh's GNI per capita is $10,080 PPP, cannot readily absorb week after week. For someone in Rangpur, Barishal, or the Chattogram Hill Tracts, distance layers onto cost.
Online group therapy in English is a specific answer to a specific shortage. Waits for a specialist can stretch to twelve weeks, and services outside Dhaka, Chattogram, and Sylhet are limited.
