Get Better, Together

Group therapy that reaches the Highlands, the coast, and every province.

Grouport connects Papua New Guineans with small, licensed-therapist-led group sessions over secure video for $32 a session, roughly K130 at prevailing rates. No PMV ride into Port Moresby, no queue at Laloki Psychiatric Hospital, no waiting on the next visiting clinician run through Mount Hagen or Kokopo. You can join a group right away and groups of six to twelve peers meet weekly in English over HIPAA-compliant video.

Dialectical Behavior Therapy

The State of Mental Health in Papua New Guinea.

Understanding the landscape of mental health care access and the challenges
residents face across the state.

Mental Illness Prevalence

11.5% of Papua New Guinean adults live with a mental health condition each year, reflecting the ongoing need for accessible therapy options.

Wait Time

14 weeks the average wait to secure a first mental health appointment in Papua New Guinea's public system, an interval during which symptoms often deepen.

Median Household Income

$2,637 median household income in Papua New Guinea, shaping what households can allocate to private mental health care.

Percentage Who Need Therapy

91.0% of Papua New Guinean adults with a mental health condition do not receive formal specialist treatment.

Provider Shortage

84% of Papua New Guinea's mental health workforce falls short of what is needed to fully meet population demand for specialist care.

Mental Health Providers per 100k Residents

8.12 per 100,000 mental health workers in Papua New Guinea, a density that shapes availability, wait times, and access across the country.

These figures are drawn from the WHO Global Health Observatory Mental Health Atlas, the catalogue, and National Department of Health reporting. They describe a country of 10.58 million residents, 84.6 percent of whom live rurally in one of the most dispersed populations on earth. The 8.12 workers per 100,000 figure aggregates every category of mental health cadre WHO recognises, from psychiatrists down to community mental health nurses; the specialist psychiatrist count on its own has repeatedly been reported in the single digits nationally.

UNDERSTANDING THE CHALLENGE

Understanding Mental Health Access in Papua New Guinea.

The Problem

Papua New Guinea's mental health system is anchored by Laloki Psychiatric Hospital outside Port Moresby, the country's sole tertiary psychiatric facility, supported by inpatient units at Port Moresby General, Angau in Lae, Mount Hagen, Goroka, and Nonga in Kokopo. The National Mental Health Policy 2020 to 2030 committed to community-based services in all 22 provinces, but the workforce to deliver that vision does not yet exist. At 8.12 mental health workers per 100,000 residents, the country's aggregate ratio sits well below the WHO Western Pacific median and far below neighbouring Australia, while the specialist psychiatrist ratio is roughly two orders of magnitude thinner.

The Impact

What that thin workforce means on the ground is that a Papua New Guinean in Wewak, Kavieng, Alotau, or the Southern Highlands who realises they need to speak with a mental health professional is typically offered one of three paths: a referral into the public system with a wait that regularly stretches to three months, a domestic flight to Port Moresby to see one of the country's handful of private practitioners, or nothing. In a nation of 600-plus islands and a Highlands interior separated from the coast by mountains that made a national road network impossible, that geography multiplies every workforce shortage by distance.

The Solution

Online group therapy is a targeted response to exactly this shape of shortage. A group of six to twelve peers meeting weekly with one licensed therapist multiplies the clinical reach of each therapist by an order of magnitude compared with one-to-one care. Because sessions run over video, the same group can include a public servant in Port Moresby, a teacher in Goroka, and a nurse in Rabaul without any of them travelling to a clinic. The model is designed to sit alongside Laloki, the provincial hospital units, and the National Department of Health's community mental health program, not to replace them.

84% of Papua New Guinea's mental health workforce falls short of what is needed to fully meet population demand for specialist care.

Grouport matches new members with a group in immediate, compared with the roughly 84-day wait typical inside Papua New Guinea's public system. Sessions are delivered in English, one of the country's three official languages alongside Tok Pisin and Hiri Motu, and the working language of Papua New Guinean universities, government, and formal-sector employment. The service is delivered by licensed therapists over HIPAA-compliant video, workable wherever the Digicel PNG or Vodafone 4G footprint reaches, which now covers Port Moresby, Lae, Mount Hagen, Madang, Kokopo, and a rapidly expanding set of provincial towns.

Access and Availability Across Papua New Guinea.

Access to mental health care in Papua New Guinea is shaped by two facts that override almost everything else: a workforce ratio of 8.12 professionals per 100,000 residents concentrated overwhelmingly in Port Moresby and the two Highlands referral cities, and a population of 10,576,502 of whom only 15.4 percent live in urban centres. Any honest description of access has to start there and work outward from Port Moresby.

Geographic Barriers

Papua New Guinea is 84.6 percent rural, one of the highest rural shares of any country on earth. That figure is not a rounding artefact, it describes roughly 8.95 million people living across the Highlands provinces, the Momase coast, the Islands region, and the Southern coast, in communities that in many cases are not reachable by road from the nearest hospital. The Highlands Highway between Lae and Mount Hagen carries much of the interior's traffic, but provinces like Western, Gulf, Enga, and the outer islands of Manus, New Ireland, and Bougainville depend on domestic flights, coastal shipping, and river transport. A resident of Daru or Lorengau needing to see a psychiatrist at Laloki is looking at a flight to Port Moresby, accommodation, and lost wages before the first consultation happens.

Extended Wait Times

Inside the public system, the 14-week wait cited on this page reflects consistent reporting from the National Department of Health and Pacific-region mental health surveys, and is the best-case figure for someone starting inside the National Capital District. In the Highlands provinces, in Momase, and in the Islands region the wait is often materially longer because there is often no resident psychiatrist at all and the visiting-clinician schedule is measured in months rather than weeks. Twelve weeks is a long time for a depressive episode, a postnatal adjustment, or a trauma response to sit untreated, and for many rural Papua New Guineans the real wait is effectively unbounded.

Systemic Challenges

Beyond raw workforce numbers, Papua New Guinea's system carries a mental health budget line that the WHO Mental Health Atlas has repeatedly flagged as a small fraction of overall health spending, sustained brain drain of trained clinicians to Australia under skilled migration pathways, and a training pipeline that graduates only a handful of psychiatric registrars per year through the University of Papua New Guinea. Layered on top are the acute mental health loads of tribal conflict recovery, sorcery-accusation-related violence, and the disaster mental health work following events like the 2018 Highlands earthquake and repeated volcanic and flooding emergencies. Wantok networks, churches, and community structures carry much of what a fuller professional system would otherwise handle.

Urban-Rural Divide

With only 15.4 percent of the population urban, the workforce that does exist clusters in Port Moresby, Lae, Mount Hagen, and Goroka. Provincial hospitals in Madang, Wewak, Kokopo, Kavieng, Alotau, and Buka extend coverage but not psychiatric depth. For a Papua New Guinean living in a Highlands village, an outer atoll of Manus, or a Sepik river settlement, the practical psychology workforce density on any given week is effectively zero. Telehealth changes that arithmetic in a specific way, because a stable Digicel 4G signal substitutes for a domestic flight to Jacksons, and the group format substitutes for a one-to-one appointment that would otherwise not exist at all.

Grouport is not the right service for a Papua New Guinean in acute psychiatric crisis, for whom Laloki Psychiatric Hospital, Port Moresby General's emergency department, the provincial hospital emergency departments, and community lifeline services remain the first calls. It is a strong fit for the moderate-severity Papua New Guinean adult, English-comfortable, who wants weekly, structured group work with a licensed clinician, delivered wherever there is a phone signal and a private corner, at a price that does not require choosing between therapy and school fees or store goods.

Affordable Group Therapy Costs for Papua New Guinean Residents.

Affordability and Income

Set against Papua New Guinea's lower-middle-income US$2,637 GNI PPP per capita, US$140 a month is a meaningful line item, and set against a single-session private psychiatry fee in Port Moresby it is often less than what one out-of-pocket appointment already costs. Because the price is flat and covers weekly sessions across the month, there is no per-visit tally to worry about, no separate charge for a longer session, and no surprise K-denominated bill at month end. For salaried Papua New Guinean professionals, mine-and-LNG-sector employees, and family members overseas supporting relatives' care, the predictability is part of the value.

Hidden Cost and Barriers

The sticker fee on a Port Moresby private appointment is only part of the true cost for most Papua New Guineans. Add the airfare from Mount Hagen, Wewak, Kavieng, or Alotau, the taxi in from Jacksons Airport, the accommodation at Port Moresby prices, the security considerations of moving through the city, and the days of lost wages, and the effective per-session cost becomes several thousand kina before a single clinician has been paid. For Highlands and Islands residents a domestic flight alone regularly exceeds K1,000 return. A weekly video group removes every one of those line items and puts the money into the therapy itself.

Immediate Availability

The immediate group start window is the other half of the value story. In a country where the practical alternative is an 84-day wait for a first public appointment, or an expensive Port Moresby private consultation that may still take weeks to schedule around clinician availability, being in a group by the end of the following week reframes what the fee is buying. It is not just a lower cost per hour, it is access on a timeline that matches the shape of an actual mental health need.

Private mental health care in Papua New Guinea is priced against a market where the supply of specialists is measured in single digits. A single consultation with one of Port Moresby's private psychiatrists or overseas-trained psychologists can run K400 to K800 or more, which for a household near Papua New Guinea's US$2,860 GNI per capita on the Atlas method is a serious weekly outlay. Grouport is US$140 a month, roughly K567 at current rates, for unlimited weekly sessions across the month.

Mental Health Conditions We Treat in Papua New Guinea.

Grouport runs weekly groups for anxiety, depression, DBT skills, grief and loss, relationship issues, OCD, PTSD and trauma processing, ADHD, chronic pain and illness, LGBTQIA+ experience, and parenting. These map closely to the presentations Papua New Guinea's provincial mental health teams most commonly document, including the trauma load carried by communities affected by tribal conflict, gender-based violence, and natural disasters across the Highlands and Islands regions, and the anxiety and mood conditions that the country's small in-country specialist workforce cannot come close to meeting demand for.

Meet Our Therapists

Our therapists represent a wide range of clinical specialties & diverse backgrounds. They all undergo the most stringent credentialing process. Grouport therapists are caring, expert mental health professionals with years of experience helping people get the tools they need to see long-lasting change.

Grouport therapists are licensed clinicians working in English, one of Papua New Guinea's three official languages and the working language of Papua New Guinean universities, government, and the formal sector. All are trained in facilitating small groups over video and are experienced with participants across many time zones, including Pacific-based clients working across the Papua New Guinea Time offset. Because the format is group rather than one-to-one, cultural context and shared experience are built by participants themselves alongside the clinician, which fits the wantok and community-oriented way many Papua New Guineans already discuss wellbeing.

Common Questions About Group Therapy in Papua New Guinea.

Mental illness carries heavy stigma in PNG and visits to Laloki or a private Port Moresby clinic are highly visible. How does Grouport protect privacy?

Grouport is fully remote, so there is no waiting room in Port Moresby or Lae where colleagues, church members, or wantok connections might notice you. Weekly sessions of 6 to 12 members happen over HIPAA-secure video with licensed therapists, from your own home or office anywhere on PNG Time. Groups are organized by concern, such as anxiety, depression, or grief, so peers understand what you are working through. At $32 per session with an immediate group start, discreet, consistent support is realistic for professionals, expatriate workers, and provincial residents alike.

In Papua New Guinea, many people first consult traditional healers or attribute distress to sanguma. How does Grouport fit alongside that landscape?

Grouport gives Papua New Guineans a clinical option that is often hard to reach locally, where Laloki Psychiatric Hospital in Port Moresby and a handful of provincial units serve 10.5 million people at 8.12 mental health workers per 100,000. Weekly video groups of 6 to 12 members are led by licensed therapists using evidence-based approaches like CBT and DBT, at $32 per session. Sessions run over HIPAA-secure video on PNG Time (UTC+10), so English-speaking clients in Port Moresby, Lae, Mount Hagen, or remote highlands can join right away.

How much does online therapy typically cost?

Grouport's pricing varies by what type of therapy you need. Group therapy is typically $25–$35 per session depending on which group you sign up for, usually billed at $140/month for weekly sessions. Individual therapy is $448/month for weekly sessions or $224/month if you do every-other-week. Couples therapy is $492/month. Family therapy is $640/month. We also offer IOP (Intensive Outpatient Program) starting at $1,348/month for people needing more intensive support. All of these are significantly lower than the U.S. average, and there's no long-term commitment, you can pause or cancel anytime.

Can therapy help with social anxiety?
Social anxiety, fear of social situations, worry about judgment, avoidance of social interaction is common to experience. Cultural norms affect what situations trigger anxiety, but the core condition is the same. Therapy uses evidence-based approaches to treat social anxiety regardless of cultural context.
What if I'm experiencing burnout?
Burnout from work, caregiving, activism, or other demands affects people globally. Symptoms are similar across cultures, exhaustion, cynicism, reduced effectiveness. Therapy helps you address burnout, set boundaries, and figure out if you need to change your situation or just change your relationship to it.
Can I suggest topics for the group to discuss?
Yes, groups are driven by member experiences. So definitely bring what’s going on in your life as that can impact the topic of discussion. The therapist facilitates the flow of conversation and what time of group focuses on what, but you and other members bring in what matters to work on and that certainly gets paid attention to and becomes a focus in conversation. By bringing in your own experiences, this makes the group more relevant and enables others to chime in and present skills that could be relevant to that specific situation.
What if I feel like I don't fit in with the group?
Give it some time. Feeling Initial discomfort is normal. If after a couple of sessions you still feel like you’re questioning whether the group is the right fit or not, talk to a care coordinator and they can help you explore what might be best for you. Maybe a different group would be a better fit, or perhaps it's worth sticking with the group to see if it's just taking some time to ease into it, which can happen before you find your rhythm with the group. You can always switch groups at any time in the event you wish to switch to another group, and a care coordinator will work with you to make sure you’re happy with your group fit.
Can online group therapy help with anger management?
Yes, anger management groups are highly effective and are a tremendous help in navigating the ins and out of anger challenges. Group is ideal for anger work because you receive honest feedback about how your anger affects others, you practice managing frustration in real situations, and you learn from others' struggles and successes, and are held accountable by your peers. In addition to general anger management groups, we also find that dialectical behavior therapy (DBT) groups are highly effective for anger as they help with emotion regulation, distress tolerance, mindfulness, and interpersonal skills like lashing out in relationships and rage.
What if I want to work on issues but don't want to share details?
You don't need to share every detail to benefit from group. You can participate meaningfully without sharing every detail of your life. You can keep it high level by talking about patterns, feelings, or what you're struggling with conceptually. Ultimately, you totally control your level of disclosure. Many people surprise themselves by sharing more than expected once trust builds. By recognizing the commonalities people experience in the group, most people ultimately feel comfortable sharing more than what they would have though they’d feel comfortable sharing. It is precisely being vulnerable with others and confiding in others, which serves as a major driver of improved therapeutic outcomes and why online group therapy is so successful.
What technology do I need for online therapy?
You’ll need a device with a camera and microphone such as a smartphone, tablet, laptop, or desktop computer along with a stable internet connection. Grouport's platform works on most modern devices and browsers. If you can video call with friends or family, you can attend Grouport therapy sessions. Many of our sessions happen within our member portal, in which case it uses our proprietary video chat technology. If the session doesn’t happen within our member portal, many of our sessions also happen over Zoom’s HIPAA compliant platform, so in that case you would have to download zoom which you can do for free.
How does online therapy work?
Online therapy with Grouport works through video sessions where you meet with a licensed therapist from the comfort of your home. After you sign up, we match you with a therapist within 24-48 hours based on your needs, schedule, and preferences. Sessions are conducted via our HIPAA-compliant video platform - you simply log in at your scheduled time and connect with your therapist. You'll receive the same evidence-based treatment and professional care as in-person therapy, with the added convenience of attending from anywhere.
What if I'm not comfortable on camera?
While video is recommended for the best therapeutic experience, you have options if you're uncomfortable on camera. For private sessions, like individual therapy, couples therapy, or family therapy that would just be private with you and the therapist, so for that video should be on. For group sessions, which include other members that you do not know personally, you can turn off your camera and use audio only, though your therapist may occasionally ask you to turn it on briefly for check-ins. Some clients start with audio only and become more comfortable with video over time, though we do recommend keeping video on as that provides for the most therapeutic benefit. You can also adjust the video settings so you don't see yourself if that helps with camera anxiety. For group sessions specifically, most members are surprised by how quickly they feel comfortable in the group setting, and report that sharing and being vulnerable with others is precisely the leading element to their recovery process. Talk with your therapist about your concerns, they can help you find a format that feels comfortable while still providing effective treatment.
