Get Better, Together

Group Therapy for Botswana, Without the 20-Week Queue.

Botswana has roughly one mental health worker for every two million residents, and public-sector waits stretch past four months. Grouport connects Motswana adults to licensed therapists right away through small English-language video groups. Sessions run at $32 each, so weekly support does not have to compete with rent, school fees, or a trip into Gaborone.

The State of Mental Health in Botswana.

Understanding the landscape of mental health care access and the challenges
residents face across the state.

Mental Illness Prevalence

13.4% of Motswana adults live with a mental health condition each year, reflecting the ongoing need for accessible therapy options.

Wait Time

12 weeks the average wait to secure a first mental health appointment in Botswana's public system, an interval during which symptoms often deepen.

Median Household Income

$5,320 median household income in Botswana, shaping what households can allocate to private mental health care.

Percentage Who Need Therapy

85.0% of Motswana adults with a mental health condition do not receive formal specialist treatment.

Provider Shortage

100% of Botswana's mental health workforce falls short of what is needed to fully meet population demand for specialist care.

Mental Health Providers per 100k Residents

0.05 per 100,000 mental health workers in Botswana, a density that shapes availability, wait times, and access across the country.

These figures pair a small workforce with a large population of people who could benefit from care. WHO's Mental Health Atlas puts Botswana's specialist workforce at 0.05 per 100,000, and World Bank population data confirms the country holds just over 2.52 million residents, so the raw math produces about 1.3 clinicians for the entire national caseload. When Southern African treatment-gap estimates are applied to that base, the shortfall becomes structural rather than incidental.

UNDERSTANDING THE CHALLENGE

Understanding Mental Health Access in Botswana.

The Problem

Botswana's mental health system centers on a handful of hospitals, Sbrana Psychiatric Hospital in Lobatse, the psychiatric unit at Princess Marina in Gaborone, and district-level clinics that lean heavily on general nurses for triage. WHO records 0.05 mental health workers per 100,000 people, which is one of the lowest ratios in the Southern African Development Community. For a country of 2.52 million, that ratio leaves roughly 32,000 residents queued behind each available clinician even before demand is factored in.

The Impact

The impact shows up in month-long referral chains. A resident in Serowe or Maun who is flagged for depression at a district clinic can wait five months for a specialist slot, often traveling to Gaborone or Lobatse for the appointment itself. Fuel, bus fare, and lost wages compound the delay. Because 32.5% of Motswana live outside urban centers, the practical wait is longer than the 12-week statistic suggests once transport logistics are included.

The Solution

Online group therapy sidesteps the two chokepoints, clinician scarcity and geography, by pooling six to twelve participants around one licensed therapist. Grouport's licensed clinicians run condition-specific groups in English, which is one of Botswana's two official languages and the medium of instruction from upper primary onward. The format lets one therapist serve a dozen Motswana adults a week without any of them boarding a combi to Gaborone.

100% of Botswana's mental health workforce falls short of what is needed to fully meet population demand for specialist care.

Grouport matches new members to a group right away instead of the 12-week public queue. Sessions run over HIPAA-compliant video, so a participant in Francistown, Kasane, or a cattle post near Ghanzi joins from a phone or laptop on the same 4G that carries WhatsApp. English delivery removes the interpreter bottleneck that limits many cross-border telehealth options, and the $32 per-session rate is billed in USD without requiring local insurance paperwork.

Access and Availability Across Botswana.

Formal mental health care in Botswana is concentrated in a small number of facilities, and demand consistently outruns supply. Even in Gaborone, home to the country's largest psychiatric unit, private outpatient slots book weeks in advance, and public referrals move on the 12-week clock. The map of care and the map of residents do not overlap well.

Geographic Barriers

Botswana covers 581,730 square kilometers, most of it Kalahari sandveld with settlements spread thin along the eastern corridor and the Okavango panhandle. A resident of Kasane in the far north sits roughly 940 kilometers from Sbrana Psychiatric Hospital in Lobatse. Bus service on the A1 is regular but slow, and there is no domestic airline linking most secondary towns. For someone managing anxiety or postpartum depression, the trip itself becomes a barrier to the first appointment, let alone weekly follow-up.

Extended Wait Times

The 12-week average wait time recorded for Botswana is not evenly distributed. Urgent cases at Princess Marina Hospital move faster, but routine referrals for depression, anxiety, and grief regularly stretch into a fifth or sixth month. Twenty weeks is long enough for a work performance issue to become a dismissal, or for a student in Molepolole to miss an entire semester at the University of Botswana. Waiting five months for a first conversation with a clinician often means the presenting problem has compounded by the time care begins.

Systemic Challenges

Public mental health services are delivered under the Ministry of Health and Wellness, mostly free at point of care but rationed by the workforce ceiling of 0.05 clinicians per 100,000. Botswana Public Officers Medical Aid Scheme (BPOMAS) and private schemes like BOMAID cover some private psychology visits, but reimbursement caps and the shortage of registered psychologists in private practice, clustered almost entirely in Gaborone, limit that pathway. Uninsured residents outside the capital face out-of-pocket private rates that rival a month's household spending.

Urban-Rural Divide

Statistics Botswana puts urbanization at 67.5%, which means about 819,000 Motswana live outside classified urban settlements. Almost every registered psychologist and psychiatrist practices in Gaborone or Francistown, so that 32.5% rural share is served overwhelmingly by general nurses trained in mental health screening rather than by specialists. For a farmer in Ghanzi District or a fisher in the Chobe, the nearest specialist is a day's travel away, and a follow-up cadence of once a week is essentially impossible without relocation.

A stable internet connection is the only geographic requirement.

Affordable Group Therapy Costs for Botswana Residents.

Affordability and Income

Botswana's GNI per capita in PPP terms is $5,320, and the Atlas-method figure is $7,610, a reminder that headline PPP hides real household purchasing power. At $140 per month, Grouport represents roughly 0.7% of annual PPP income and about 1.8% of annual Atlas-method income. Compared to a single private session in Gaborone, one month of weekly Grouport groups often costs less than two local visits.

Hidden Cost and Barriers

Local therapy costs do not end at the consulting-room door. A resident of Selebi-Phikwe attending a monthly appointment in Gaborone spends roughly P400 on round-trip coach fare, another P200 on food and incidentals, and a full day away from work. Weekly attendance compounds those numbers fast. Grouport removes the transport, the annual leave, and the childcare arrangement, sessions are attended from home in the evening slot that suits the participant's schedule.

Immediate Availability

New members are matched to a group right after enrollment. That is roughly 100 times faster than the 12-week wait tracked by WHO for Botswana's public services, and it means the decision to seek help translates into a session on the calendar the same week.

Private psychology visits in Gaborone typically run between P600 and P1,200 per session, roughly US$45 to US$90 at current rates, and are billed weekly. For most households outside the top income deciles, that cadence is unsustainable without medical aid. Grouport's group format brings the per-session cost down to $32, or about P425, while keeping the weekly rhythm that makes therapy work.

Mental Health Conditions We Treat in Botswana.

Grouport runs groups for the conditions that show up most often in Southern African primary care records, depression, generalized anxiety, social anxiety, panic, obsessive-compulsive patterns, PTSD and complex trauma, grief and bereavement, relationship distress, and dialectical behavior therapy skills for emotion regulation. Groups are matched by presenting issue rather than by geography, so a Motswana participant sits with peers working on the same challenge, facilitated by a licensed clinician trained in the relevant modality.

Meet Our Therapists

Our therapists represent a wide range of clinical specialties & diverse backgrounds. They all undergo the most stringent credentialing process. Grouport therapists are caring, expert mental health professionals with years of experience helping people get the tools they need to see long-lasting change.

Grouport's clinicians are licensed, and work with Botswana members over HIPAA-compliant video in English. Sessions are conducted in the same working English used across Botswana's civil service, universities, and business sector, which removes the interpreter step that limits many cross-border teletherapy options. Facilitators are experienced in leading small groups across time zones and cultures and adjust pacing so participants have room to bring in local context, work culture, family structure, or church community, without translation.

Common Questions About Group Therapy in Botswana.

How does Grouport compare to seeing a private psychologist in Gaborone through BOMAID or Pula Medical Aid?

Private sessions with the handful of clinical psychologists in Gaborone typically run 800 to 1,200 pula per hour, and new-patient waitlists commonly stretch around 20 weeks because specialists are concentrated in the capital and Francistown. Grouport's weekly video groups cost about 435 pula per session at $32, are led by licensed therapists, and let you join right away with an immediate group start. You skip the drive into the CBD, avoid running into someone you know at the clinic, and get consistent weekly support from home.

I work long hours in Gaborone's mining, NGO, or diplomatic sector. Can Grouport fit around a Botswana professional schedule?

Yes. Grouport runs weekly video groups of 6-12 members with evening slots that work well from Central Africa Time, so you can join after a shift at Debswana, a UN agency, or a Gaborone law firm without leaving the office or sitting in Western Bypass traffic. Sessions are led by licensed therapists, run entirely in English, and stay HIPAA-secure and private, which matters when colleagues share the same medical aid schemes. At $32 per session you can join right away and start the same week.

Do therapy costs vary by therapist credentials?
Sometimes. Psychiatrists (MDs) often charge more than licensed therapists. Among therapists, rates vary more by experience, location, and specialization than by credential type (LCSW vs. LPC vs. LMFT). There's no universal pricing based on credential letters.
Does Grouport accept international insurance?
We don't directly accept insurance from any country currently. You pay out-of-pocket and can request a receipt to submit to your insurance if they offer reimbursement for mental health services. Coverage varies widely by country and insurance provider so you’d have to check with them about reimbursement.
How do time zones work for international clients?
Our therapists are located all over the world and work various hours to accommodate different time zones. Depending on where you live, sessions might be in your morning, afternoon, evening, or night. We'll work with you to find times that fit your schedule and our therapists' availability.
What if group members give conflicting advice?
Multiple perspectives can be actually valuable. You're getting different viewpoints, then deciding what resonates with your situation. First consider what others are saying, and then find what works for you. Like anything in life, take what helps for you, and anything that’s unproductive disregard.
Is group therapy confidential?
Yes. Everyone in the group agrees upfront that what's shared in group stays in group and everyone in the group is there for a similar reason. The therapist maintains professional confidentiality just like in individual therapy. However, all group members must also agree to confidentiality as what's shared in group stays in group. Most group members take confidentiality seriously because they're also sharing vulnerable information and want the same protection. Many people find they're comfortable sharing in online group therapy sessions because others genuinely understand and respect the need for privacy. Keep in mind after all the members are there for the same reason you are.
What if I don't want to hear about other people's problems?
This concern is understandable as when you're struggling, it seems counterintuitive that listening to others' problems can help you. However, online group therapy works differently than expected as hearing others describe similar struggles reduces isolation and shame, observing others' progress provides hope and practical strategies that you can implement, giving support to others builds confidence and purpose, and different perspectives help you see your situation more clearly, and witnessing others' challenges often provides needed perspective on your own. Online Group Therapy isn't just venting about problems, but rather it's active problem-solving and skill-building together. If group feels like it's dragging you down rather than helping, perhaps it’s a matter of the group fit and it's worth exploring a different group fit, or individual therapy might be a better fit. But most people find that supporting and learning from others is surprisingly beneficial, and that is why the majority of our members do online group therapy as a primary part of their treatment plan.
What if I need to contact the therapist between group sessions?
For most things you need in between sessions, contact a care coordinator and they will be able to assist you with most things or point you in the right direction. The therapist of the group isn't intended for individual contact in between sessions for the most part, but if it's an extenuating circumstance of course your care coordinator can help put you in touch with them if it's an extenuating situation. Most questions you need outside of session, should be able to be addressed by a care coordinator and accelerated to the group therapist in an extenuating circumstance. If you need individual support, then a care coordinator can get you set up with individual therapy with either your group therapist or another qualified therapist who is a good fit for you.
How do I get started with Grouport’s online therapy?

Getting started is easy. First, visit grouporttherapy.com and click "Get Started." This takes you to the service selection page, where you choose which type of therapy you are interested in and complete a brief intake form about your goals and preferences. We then match you with a licensed therapist or group based on your needs and any specific requests. After signing up, a care coordinator reaches out by email or phone to walk you through available therapists and scheduling. You make the final choice about your care, including which therapist you meet with and when. Once confirmed, you attend your sessions weekly over video chat.

Can I attend online therapy sessions via phone if needed?
Yes\! You can attend over video chat on any smartphone. While we recommend video on a computer or laptop for the best therapeutic experience, you can attend sessions by any smartphone as well. Additionally, you can also attend sessions by audio only if needed, though we recommend to join by video for the best experience.
What internet speed do I need for online therapy?
A stable internet connection of at least 3 Mbps is recommended for video sessions. If video connection isn't working well for some reason, you can always switch to audio-only during the session.
